¡Hola!
The problem comes because "fue" means "it was" and "it went", but in this case you can translate it just as "it went".
All the questions are correct. This is what they mean more or less:
1)How did your weekend go?
2)How was your weekend?
3)How did the weekend go to you?
Althought I would say: ¿Qué tal el fin de semana?

The other two are also correct, but remember that if you use "fue" in the example a) you mean "How did your day go?" and with "estuvo" it means "How was your day?

I have heard both uses and wonder which is correct. Could someone please help?

1) Como fue tu fin de semana?
2) Como estuvo tu fin de semana?
3) Como te fue el fin de semana?

Also, extending that same idea how would I say..

How was your day? How did you do on the exam?

a) Como fue/estuvo tu dia?
b) Como te fue en el examen? Como estuvo el examen
