The problem comes because "fue" means "it was" and "it went", but in this case you can translate it just as "it went".
All the questions are correct. This is what they mean more or less:
1)How did your weekend go?
2)How was your weekend?
3)How did the weekend go to you?
Althought I would say: ¿Qué tal el fin de semana?
The other two are also correct, but remember that if you use "fue" in the example a) you mean "How did your day go?" and with "estuvo" it means "How was your day?
